Question 1

What do I need to bring to my appointment?

Accepted Answer

You should bring your photo ID, all medical and vision insurance cards, and any glasses you use, including computer glasses, sunglasses, over-the-counter readers, and any other special lenses you use regularly. If you wear contact lenses, please bring your current boxes as well.

Question 2

How do I access the patient forms?

Accepted Answer

The patient forms are available through an online patient portal. You can click on "Access our Patient Forms" and then "Let's Start" to access the portal. If this is your first time on the portal, you don't need to enter a code, password, or email address to log in. Once logged in, you can create a password to access the forms in the future.

Question 3

What information do I need to provide on the patient forms?

Accepted Answer

It is important to fill out all information boxes thoroughly, even if you have been a patient at the practice for years. This includes providing a full medical history and listing all medications you take, including prescription, over-the-counter, supplements, anti-inflammatory medications, infusions, injections, inhalers, and eye drops.
