Inclusa is a managed care organization based in Stevens Point, Wisconsin, United States. Established in 2000, Inclusa's mission has been to maximize health outcomes for its members by partnering with others to build vibrant and inclusive communities. Recently, Humana has agreed to purchase substantially all of Inclusa's assets, allowing them to create a model of care that provides industry-leading support for members across the healthcare continuum.
